Patient and operation

Original fictional record. Allan Pierce, age 71, undergoes a scheduled open left carpal-tunnel release in an outpatient operating room on September 9, 2026. The signed operative report documents completion of the release. The anesthesia professional claim is being prepared for Original Medicare.

Anesthesia assessment and plan

The anesthesiologist records the patient’s history and assigns ASA II after evaluation. The signed plan uses a left upper-extremity regional block as the operative anesthetic, with monitoring and supplemental sedation. The plan explicitly states that surgical anesthesia depends on the block. General anesthesia is not separately furnished in this record.

Block and operative records

Before incision, the anesthesiologist documents placement of the block, the left-sided site, the technique and the response. The block produces the intended surgical anesthesia and the operation proceeds under that plan. The operative note and anesthesia note describe the same intervention; there is no second block procedure.

Recovery and proposed charge

In recovery, the patient reports that the left hand remains comfortable. The note describes the continuing effect of the operative block. The surgeon has not requested an independent postoperative pain procedure, and the anesthesiologist has not documented another intervention. A proposed extra line labels the original block as postoperative pain management because its benefit persisted into recovery.

Review task

Decide whether that proposed extra line represents a distinct service. In your book, identify the relevant anesthesia and block instructions, but do not infer a separately furnished procedure from the duration of relief. Keep the operative anesthetic’s purpose and the later recovery finding in separate worksheet fields.
