An unplanned but related return to the operating room

Read an original fictional chart, identify the supported facts, and explain your coding workflow.

Service date: 2026-09-09 · Medicare postoperative professional service

Use your code books and write your reasoning before opening the walkthrough. This exercise contains no real patient information.

Original operation

A fictional adult underwent an operation earlier in August. The selected operation is stipulated to have a ninety-day Medicare global period. The same surgeon remains responsible for postoperative care.

New presentation

During that period, the patient develops a documented postoperative problem at the operative site. The surgeon expressly relates the problem to the original operation.

Return procedure

The surgeon takes the patient back to the operating room for an unplanned procedure to address that related problem. The report identifies the room, procedure, findings and relationship to the earlier operation.

Relationship statement

This is neither a planned stage nor treatment of an unrelated condition in the supplied record. The current exercise asks which postoperative relationship governs modifier review, with exact procedure selection left to the complete authorized entries.

Your coding worksheet

  1. Place the return within the supplied global period.
  2. Identify whether the return is planned, related or unrelated.
  3. Explain why unplanned does not mean unrelated.
  4. Locate the applicable related return-to-room procedure instruction.

In your notebook, record the supported facts, your index route, the instructions you checked, and any missing detail that limits your answer.

Write your answer

Use only this fictional record. Your entries stay on this page and disappear when you leave or reload. Do not enter real patient information.

Compare with the worked explanation
  1. The current procedure occurs within the stipulated postoperative period.
  2. The clinician explicitly relates it to the original operation, and the return is unplanned.
  3. Unexpected timing does not remove the clinical relationship.
  4. Review the related return-to-operating-room pathway rather than the unrelated postoperative procedure pathway, then verify all applicable instructions.
