A fictional adult has closed fractures of two metatarsal bones in the right foot. The treating clinician confirms both injuries and documents the individual sites. Both are managed without manipulation at this encounter.

Treatment plan

The physician supplies one closed-treatment plan using a single immobilization device for that foot and assumes follow-up. No fracture in this record undergoes manipulation, percutaneous fixation or open treatment. The exercise stipulates that both fractures fall within the cited shared-treatment policy.

Documentation distinction

The chart preserves two fracture diagnoses. A preliminary procedure worksheet duplicates the same closed-treatment-without-manipulation service solely because there are two diagnoses. The learner must apply the procedure-unit rule while retaining the supported injury details.
